Bridges to Health Equity
In 2020, LISC NY launched the Bridges to Health Equity program, which is a multi-sector collaborative effort, including a cohort of community-based organizations; managed care organizations (MCOs), and health care providers, along with the NYC Department of Health and Mental Hygiene (NYC DOHMH) and the New York Academy of Medicine (NYAM). The program provides families with asthma with health education, resources to improve their housing conditions and remove asthma triggers, and connections to other needed services.
The program is delivered through a collaborative, multi-sector approach, where all partners cross-refer participants and regularly share information to reach common outcomes. Our goal is to pilot this new structure for service delivery and secure Medicaid as a core-funding stream. Partners will then be able to use this structure to implement other kinds of programs and strategies, which impact social determinants of health.
In the pilot year, the community partners collectively provided 134 New Yorkers with home-based asthma services combined with COVID-related education and referrals to social services. In 2021, LISC NY is continuing to support these services and is working with the community partners to finalize a contract with at least one Managed Care Organization to establish a sustainable financial stream for these services through Medicaid.
