Pawtucket Central Falls Health Equity Zone
Because we believe that your zip code shouldn’t determine your health, LISC supports the work of our HEZ community partners to improve the health of our neighbors. Together, the PCF HEZ supports increased access to healthy food, improved access to health care, and revitalized and safe places to exercise and enjoy our community — so all of our neighbors have a chance to reach their highest health goals.
As the backbone entity for the Pawtucket Central Falls Health Equity Zone (HEZ), an initiative developed by the Rhode Island Department of Health (RIDOH) in 2015, LISC has engaged the community, gathered a strong and collaborative group of partners, and created a unified approach to finding strategies to address health concerns and challenges in Pawtucket and Central Falls.
During the past several years, the community challenges have evolved and new ones took center stage, but the main philosophy has remained a priority — that prioritizing community input can help achieve tremendous results. We've gathered nearly 150 unique partners in the HEZ network with more than 40 active participating organizations - all working toward the same goal: improved health equity for our community.
